Chinese translation document: Survey on the understanding and using of medical data base for doctors

Five years have been passed since the foundation of EBM promoting group in July 1996 and establishing of evidence medicine center in July 1998 for endeavoring to popularize the evidence medicine in Zhanghua Christianity Hospital. The aim of this study was to observe the situation of the understanding and using of medical data base in the doctors in Zhanghua Christianity Hospital, and we hope to discover some insufficiency points in the medical data base education aspect from this survey, and make reviewing and improvement according to the result.
 
Questionnaire method was adopted in this study, and the subjects were the attending physicians, residents and interns in totally 482 members. The interviewees filled the questionnaire by themselves. The term of questionnaire issuing and collecting was in three months from March 1st to May 31st in 2001. The contents of the questionnaire had three main parts: 1. Basic information including birth month and year (age), duty month and year (past service), department, position and sex ect.; 2. Data base: inquiring the interviewee item by item the familiar and using status to internet and the three data base Cochrane libraryPubMed-free medlineOvid (medical data base system purchased by this hospital); 3. Evidence medicine: to find the familiar and attaining status of interviewee to related organization and curricula for evidence medicine pushed by this hospital.
 
The number of valid questionnaire collected was 247 with the replying rate as 51%. The study results showed that: 1. For using status of internet, there were 98.4% interviewee indicating “ capable of using”, and there were 97.6% saying “have means to use it in the hospital”; 2.The popularity rate, approachability and using rate of PubMed-free medline was higher than that of Ovid and Cochrane; 3. The rate of replying “knowing Cochrane library” in longer past service interviewee was significantly higher than that of with less past service (with P value as 0.002<0.05); 4. The rate of replying “have the experience of using Cochrane library” in longer past service interviewee was significantly higher than that of with less past service (with P value as 0.005<0.05); 5. For the question of “if have means to get Cochrane library available in needing”, the rate of the attending physician replying “yes” is significantly higher than that of resident physician and interne ( with P value as 0.040 <0.05); 6.The rate of expressing “have means to use internet in the hospital” , the rate of showing “knowing cochrane library”, the rate of replying “have means to get Cochrane library available in needing”, the rate of manifesting “have the experience of using Cochrane library” and the rate of indicating “have means to get PubMed-free medline available in needing” for interviewees who have joined the curriculum related with evidence medicine, were all significantly higher than those who have not the experience of evidence medicine related courses (P value<0.05).
 
Conclusion: 1. The doctors in this hospital mostly had the ability of using internet medical data base; 2. The matters of equipment and policy could influence the approachability and knowing of the medical data base; 3. At present the directions we are endeavoring to are: the short term target is to elevate the approachability of the medical data base using, and the median and long term object is to hope exerting the functions of internet medical database in the future, further using it as a great assistance to continuing education, tele-education and self-training for primary level physicians with its characteristic is not easy to be influenced by space and time.

医师对医学数据库的认识与使用调查
 
  彰化基督教医院自19967月成立”EBM推动小组19987月设立证据医学中心,努力推广证据医学,至今已逾五年。本研究之目的即是想要了解目前彰基院内医师对医学数据库的认识与使用之情形,希望藉此调查以了解本院推行医学数据库教育不足之处,据以改进检讨。

    本研究采问卷方式进行,对象为全院之主治医师、住院医师及实习医师共482人。由受访者自行填写问卷。问卷发放及回收期间自民国九十年三月一日至民国九十年五月三十一日止,共计三个月。问卷内容分三大部分:(一)基本资料:包括出生年月(年龄)、到职年月(年资)、科别、职级与性别。(二)数据库:针对Internet及三个医学数据库Cochrane libraryPubMed-free medlineOvid(本院所购买之医学数据库系统)逐项询问其认识与使用之情形。(三)证据医学:了解受访者对本院院内推行证据医学相关组织与课程的认识及参与情况。

    回收之有效问卷共247份,回复率51%。研究结果(1).针对Internet之使用状况,有98.4%受访者表示会使用,有97.6%表示有办法在医院使用(2). 使用PubMed-free medline 的普及率、可近性与使用率都高于OvidCochrane library(3).年资越久的受访者回答知道Cochrane library”的比例显著高于年资较少的受访者(P0.002<0.05)(4).年资越久的受访者,回答使用过Cochrane library”的比例显著高于年资较少的受访者(P0.005<0.05)(5).对于如果想使用时,有办法使用到Cochrane library ”,主治医师回答可以的比例显著高于住院医师及实习医师(P0.040 <0.05 )(6).曾经上过证据医学相关课程的受访者,表示有办法在医院使用Internet”的比例、表示知道Cochrane library”的比例、想使用时,有办法使用到Cochrane library的比例、使用过Cochrane library”的比例、想使用时,有办法使用到PubMed-free medline”的比例,都显著高于不曾上过课程的受访者(P<0.05)

结论. 本院的医师大都具备网络医学数据库使用之能力。二. 设备与政策之问题会影响医学数据库使用的可近性与认识。三.目前我们努力的方向 短程目标是提升医学数据库使用的可近性;中长程目标则希望将来能发挥网络医学数据库,使用其不易受空间时间限制的特性,进一步作为基层医疗医师继续教育远距教学及自我进修的一大助力。
